Once you have received confirmation of your camp registration, download and complete the Camp Health & Waiver Forms below. Please follow the instructions that are included in the forms document.
If your child's camp is not listed below, then you will need to submit the downloadable form by mail.

The Mass Audubon camps listed below use the CampDoc.com system, which allows parents/guardians to complete camp health forms and waivers securely online. If you have any questions about the system or haven't received an email from CampDoc.com after 2 weeks post registration, please contact your Camp Director.
If your child’s camp is included in the below list:
- You will receive an e-mail from CampDoc.com when it is time to complete your camp forms.
- Once completed, these forms can be used at all Mass Audubon camps listed above.
- If your camper will also attend another Mass Audubon camp not listed, you can print a copy from CampDoc.com to send to any other Mass Audubon camp that is not listed above.
- Campers attending Wildwood camp will be asked to provide additional information as part of their profile in order to fulfill overnight camp requirements.

Financial Aid Application
To apply for financial aid, download and print the form below. All completed applications should be returned directly to the camp(s) you hope to attend.
→ Please send your financial aid application materials by postal mail—for identity protection reasons, we cannot accept applications by fax or e-mail.
