Wildwood Camp Forms

The Wildwood camp forms and handbook are sent by postal mail or e-mail to parents/guardians of all registered campers each year, beginning in March. Electronic versions of the 2012 camp forms and handbook are available for download here.

The forms for Wildwood are different from the ones you may complete for a Mass Audubon day camp. If you have questions about information that will be required to complete camp forms, or how to complete the camp forms for any Wildwood program, please call the Wildwood office at our toll-free number, 866-627-2267, or e-mail wildwood@massaudubon.org.

Wildwood 2012 Handbook: The Wildwood handbook contains information for all of Wildwood’s camp programs, including overnight camp, teen adventure trips, leaders-in-training, and family camp. All Wildwood campers and their parents/guardians should review the information in the handbook together, prior to completing camp forms.

For Overnight Camp, Leaders-in-Training, and Teen Adventure Trips:

Camper Health History (4 pages): A parent/guardian should complete the health history for each camper. Include a copy of the front and back of the camper’s health insurance card, as well as copies of prescription medication orders, immunization records, asthma or allergy action plans, and any other needed information.

Camper Physical Form (2 pages): A licensed healthcare provider should complete this form for each camper. A signed form from a previous annual, school, or sports physical may be submitted in place of this form, if all the requested information is provided. This form can also be used by a prescribing physician to order prescription medications or to give permission for a camper to carry and self-administer his/her rescue inhaler or EpiPen.

Camper Form Pack (6 pages, for overnight camp only): A parent/guardian and camper should complete these forms together.

Teen Camper Form Pack (6 pages, for teen adventure trips and LITs only): A parent/guardian and teen camper should complete these forms together.

Camp Store Deposit (1 page, for overnight camp and LITs only): A parent/guardian should complete this form to set up a camp store account for a camper, if desired.

For Family Camp:

Family Camp Health History (2 pages): A parent/guardian should complete a health history for each member of the family group attending family camp.

Family Camp Form Pack (4 pages): A parent/guardian should complete one form pack on behalf of the family group.
